Receiving your Scholarship Check

Please note: You must complete this form and send your digital senior photo before your check can be mailed. The check will be payable to both you and the school you will attend. The Foundation will mail the check to you at the address you provide. Since the check will be made out to both you and your school, you will need to sign the back of the check before you mail it to your school’s bursar or tuition office. Indicate the date you wish to receive the check. Allow 2 weeks for the Foundation to process your request. To prevent spam, you will be required to enter a Scholarship Code on this form. You will find that code in your Scholarship Recipient Instructions letter. If you have misplaced that letter, email scholarship@unioncountyfoundation.org.
