Requesting Your Renewable Scholarship Check

Please note:

You must complete both parts of this form and send your college transcript before your check can be mailed.
The check will be payable to both you and the school you are attending.
- The Foundation will mail the check to you at the address you provide. Since the check will be made out to both you and your school, you will need to sign the back of the check before you mail it to your school’s bursar or tuition office.
Indicate the date you wish to receive the check.
- Allow 2 weeks for the Foundation to process your request.
To prevent spam, you will be required to enter the Renewal Code RENEWAL in Part 2.

Part 1: Get Renewal Requirements

Please select ALL scholarships for which you are requesting checks. After you click SUBMIT, you will be able to complete a form with your current contact information, and to upload your transcript showing that you have met the requirements for renewal. The specific requirements for your scholarship(s) will be shown on that form.
